THE LICENSEE NOTIFIED THE TEXAS NATURAL RESOURCE CONSERVATION
COUNCIL AND INTENDS TO NOTIFY THE TEXAS PARK AND WILDLIFE
COMMISSION REGARDING A FISH KILL IN SQUAW CREEK RESERVOIR. ABOUT
50 DEAD FISH WERE OBSERVED. THE CAUSE OF THE FISH KILL IS NOT
KNOWN. THE LICENSEE INFORMED THE NRC RESIDENT INSPECTOR.

Electrical problem ( inverter) tripped FW & condensate pumps.

Loss of spent fuel pool cooling for 20 hours unnoticed by control room operators until a senior NRC inspector detected the failure. Pool temperature rose only about 5 °F in the 20 hours. It would have taken many more days to reach 150°F at which point it would have set off plant alarms.

During test, 5 of 8 CCW check valves to RCP thermal barriers failed to stroke close, because of corrosion product accumulation.

HPCI: ultrasonic examination of the centrifugal charging pump piping found hydrogen voids which could have caused the pumps to become gas bound.

Error existed since initial operation: HPCI system suction vent isolation valves were installed backwards and would not adequatly seal against leakage when closed. Could result in common mode failure of the HPCI pumps due to gas binding.

6 valves in the containment airlock hydraulic operating system had been incorrectly classyfied: these normally open valves could have allowed eycessive containment leakage. Error existed since initial operation.
